Medical Case Management
- Address complex clinical needs
- Work independently; able to plan your own schedule with minimal oversight
- Organize a long-term plan of medical care
- Proactively compose a written plan before seeing a provider with a list of questions and concerns. Summarize results both during and after the meeting.
- Actively liaise with physician offices; respond to emails and calls in a timely manner
- Organize prescriptions including keeping a current list of generic and brand names
Caregiving
- Help with ADLs, but *not* with lifting / transferring
- Put a smile on the client's face :-)
- Provide comfort and distraction if client is in pain
- Engage with, provide structure to, and entertain client by reading stories, playing games, and other such activities
- Consistently think through contingency planning, such as what to do when the client has a temporary increase or decrease in symptoms
- Organize daily schedule and weekly/monthly calendar; plan ahead and create back-up plans
- Manage social and educational needs of the client
- Consider what activities are appropriate in light of the client's autism and how such activities may be modified to maximize the chance of success
- Proactively surmount hurdles associated with autism for the client, such as liaising and advocating for the client when they participate in community activities
- Help the client with social skills, in small steps, without unrealistic expectations
- Assist with non-medical phone calls / email / text such as ordering takeout over the phone
- Use time efficiently (such as when the client is resting or otherwise engaged) to read, research plan, write, summarize
Home Environment Organization
- Liaise appropriately with family
- Develop and implement safe home strategies such as grab bars and other assistive devices
- Purchase necessary equipment
- Adhere to a budget
- Keep a list of things to do and update daily
- Manage secure electronic information
- Cooking, meal prep, and clean-up
- Housework
Travel & Transportation
- Parking available; reliable transportation required
- Valid driver's license and clean driving record; generally speaking a car will be provided for you during the day to provide transportation for the client
- Drive client to and from medical and other appointments
- Comfortable planning multiple routes and parking together with contingencies to deal with the Boston area's notoriously difficult traffic
- Approximately once a year it will be necessary to go with the client and sometimes their family to a series of doctors' appointments out of state. Overtime pay, accommodation, and food will be provided
Training
- Extensive materials will be provided to educate a newly hired person in the client's disease process
- Hands-on training regarding details of ADLs
